ALUMNI and THIRD PARTY REQUESTS:
HVRHS alumni and third party organizations requesting a transcript or education verification should contact Mrs. Kristina Simonds, Registar, in the School Counseling Office by email at [email protected], by fax (860) 824-0130, or by telephone (860) 824-5123 extension 1170. Third party organizations must email or fax the request with the alumni release to [email protected] or (860) 824-0130. Please allow up to 10 days to process and provide the following information:

Students requesting a transcript can download this form and submit it to the School Counseling Office. Transcript Request forms are also available in the School Counseling Office. Please allow up to 10 days for processing.
SCHOLARSHIPS:
Students requesting a transcript be sent to a scholarship must submit the request in person or by email to the School Counseling Office. If the School Counseling Office is responsible for submitting your application please provide all completed materials to be submitted together. The transcript request form submitted for post graduation institutions covers all scholarship transcript requests. Please allow up to 10 days for processing.

- Your full name at the time of graduation or withdrawal.
- Date of birth.
- Year of graduation or withdrawal; specify if you graduated or withdrew.
- Where your transcript should be mailed, emailed, or faxed.
- Your telephone number/email address in case we need to reach you.

Students requesting a transcript can download this form and submit it to the School Counseling Office. Transcript Request forms are also available in the School Counseling Office. Please allow up to 10 days for processing.
- Indicate which college you would like your transcript to be sent.
- Indicate whether or not you are using the Common Application
- Add application deadline and indicate Early Decision, Early Action, Regular Decision, or Rolling Admission.
- Include the names of the individuals writing recommendations.
- Bring the completed form directly to your School Counselor.

- Indicate which scholarship you would like your transcript to be sent, along with the address.
- Indicate the scholarship deadline.
- List the names of the individuals whose recommendations you'd like to include if needed.
- Bring all materials paper clipped together to the School Counseling Office. No envelopes or staples please.
